README.md 全面更新：

  1. 快速安装 → pip install -e .
  2. 快速开始 → 统一 CLI 入口优先（bixiweave bootstrap list/align/review/status）
  3. 解耦 Demo 降级为"旧示例"副章节
  4. 项目结构 → 新增 bootstrap/ 和 cli.py
  5. 新增「三轮元评审」章节 + 共识结论 P0
  6. 路线图 → 指向 bootstrap/ROADMAP.md
  7. Wiki 链接 → 新增 Bootstrap 自举页面
